This is a unique job opening as a Guardianship Case Manager position within the Services for Older Adults Division at JF&CS. Come join our energetic team where you can use and build upon skills in serving older adults in our community, advocate for client rights and ensure vulnerable individuals are cared for with compassion and dignity.
This is a full-time position with great benefits. We offer hybrid employment, working both independently and as part of a dynamic 7-person team.
About the JF&CS Guardianship Program
For more than 30 years, the JF&CS Guardianship Program has provided customized and comprehensive case management to a diverse range of older adults no longer legally competent to make decisions regarding their health and welfare. Operating out of our Waltham location, we serve as legal guardian, appointed by the Probate Court, for those with Alzheimer's disease and other dementias, chronic mental illness, brain injury, and developmental disability. We work with those living independently in the community and those who need committed assistance.
Our goal is to provide services for the whole person - physical, emotional, and spiritual. Therefore, our work is individualized and culturally sensitive, responsive to clients' own values, religions and preferences. Each client is assigned to a social worker who gets to know the person and manages every aspect of the case. We are big enough to provide complete coverage yet small enough that each of us is familiar with all the cases.
As guardians, we take our responsibilities seriously, caring for our clients' well-being in every way, guiding them through aging and, eventually, through end of life. This work is challenging, impactful, and profoundly meaningful.
A Sampling of What a Workday Would Look Like for the Guardianship Case Manager:
- Create and manage client-centered plans of care and documentation.
- Conduct psychosocial assessments, monthly client visits to monitor the status of clients living at home, in assisted living or long-term care facilities and monitor medical and personal care.
- Arrange for or provide services including, but not limited to, applying for government benefit programs, obtaining medical or health care, exploring/obtaining alternative living arrangements, helping address housing problems and offering advocacy assistance.
- Work collaboratively with outside professionals such as Elder Protective Services, legal services, community agencies, medical & personal care providers.
- Consult with the program's legal counsel and attend court hearings on an as-needed basis.
- Document all interactions, prepare timely reports, and maintain client files in accordance with Guardianship Program guidelines.
- Participate in intake meetings, case consultations, in-service, and staff meetings.
- Be available for emergency consultation and on-call.
- Attend monthly meetings in our Waltham office as required and be an active team member!
The Benefits:
JF&CS supports employees with generous benefits. This is a full-time position with a salary range of $55,000-$60,000.
- Paid time off includes ample sick time and 22 days of vacation, in addition to national and some Jewish holidays.

Requirements
What You Bring to the Guardianship Case Manager Position:
- Bachelor's Degree with 3+ years related work experience in social services or 8+ years relevant work experience
- A valid driver's license and a reliable, insured vehicle required.
- Case management and experience working with older adults preferred
- Knowledge of community resources, entitlements, and health insurance
- Commitment to preserving the dignity of older adults
- Stellar interpersonal and communication skills and a willingness to learn
- Proven ability to manage several projects at once and respond to tight timelines
- Flexibility and initiative to manage crisis and provide ongoing care management
- Demonstrated problem solving skills with high attention to detail
- Proficiency with Microsoft Office Suite, especially Word and Outlook
