Unit Summary
Hall 2 is a50-bed long-term care and memory care unitthat provides residents with daily activities and a comprehensive range of services and support. This unit serves residents withvarying levels of acuity, from high-functioning individuals to those who are bedbound. Common diagnoses includediabetes, cardiac conditions, dementia, and behavioral health needs. Staff members on Hall 2 are expected to deliverpatient-centered, compassionate carewhile supporting both medical and cognitive needs.
Job Summary
Under the supervision of the Registered Nurse and/or Director of Nursing, the Licensed Practical Nurse (LPN) utilizes a general understanding of nursing principles and basic physical assessment skills to develop and implementindividualized nursing care plans. The LPN supports daily unit operations on a per-shift basis and may assumeleadership or charge responsibilitiesas assigned to ensure safe, effective, and compassionate resident care.
Essential Duties&Responsibilities
Provide direct nursing care to long-term care and memory care residents in accordance with established care plans
Perform basic physical assessments and monitor resident status, reporting changes in condition
Administer medications and treatments as prescribed and within scope of practice
Support residents with chronic conditions and cognitive impairments, including dementia and behavioral needs
Accurately document care and observations in the electronic medical record
Collaborate with interdisciplinary team members to promote resident well-being and quality of life
Support daily unit operations and workflow
Serve in a leadership or charge role as assigned
Adhere to safety standards, infection control practices, and regulatory requirements
Minimum Qualifications
Graduate of an accredited school of nursing (diploma program)
CurrentLicensed Practical Nurse (LPN)licensein the State of Maryland
AHA BLS Certificationrequired
Less than one (1) year of nursing experience required
Formal working knowledge equivalent to an Associate's degree (two years of college)
Knowledge of nursing practices appropriate for long-term care and memory care populations
