Frasier is hiring for a full time MDS / RAI Coordinator. This position will be on-site, full-time
Here's what you'll do:
Lead and coordinate the Resident Assessment Instrument (RAI) process across disciplines
Complete and submit Minimum Data Set (MDS) assessments accurately and on time
Collaborate with nursing, therapy, and other team members to gather and validate information
Ensure compliance with CMS and state regulatory requirements
Monitor assessment schedules and follow up to keep everything on track
Support care planning by ensuring assessments reflect each resident’s needs
Serve as a resource to the team on MDS and RAI requirements
Participate in on-call rotation responsibilities

Compensation
Pay Range: $33.30 - $42.45 per hour
This position will be on-site, full-time

Requirements
Current, active RN license in Colorado (in good standing)
At least 3 years of experience as a Registered Nurse, including 1+ year with MDS completion
Solid understanding of Medicare, Medicaid, and state/federal regulations related to MDS
Working knowledge of ICD-10 coding
Experience with electronic health records (PCC experience a plus)
Strong computer skills and comfort with clinical documentation systems
Clear communication skills and the ability to work effectively with residents, families, and team members
