- Oncology diagnosis, Dementia/Alzheimer's, end stage CHF/COPD, end stage neurological conditions (ALS, MS, Parkinson's)
- Patient Ratios:
- Case Manager averages 15 to 20 patients
- Required Licensure & Certifications:
- RN Licensed for State of Maine
- BLS
- Driver's License? Yes, driver's license and must use own vehicle to drive from patient home to patient home.
- Skills required:
- Nursing skill set to care for the common diagnosis listed above
- Support within the Department:
- Scheduling
- Clinical Manager
- Lead Support Team
- Technology/Equipment:
- EMR: Netsmart
- IV pumps: Provided through vendor
- Floating:
- If South Portland Territory Census is low may ask traveler to support other agency locations, if census allows.
