As our Assessment RN, you will complete in-home intake assessments, identify clinical and safety risks, and create clear care plans and caregiver instructions. You’ll help ensure our caregivers deliver consistent, safe support aligned with the client’s needs and family expectations.
Key Responsibilities
Conduct in-home assessments (health, function, safety, environment, supports) Create individualized care plans and caregiver task lists (clear, measurable) Complete medication reconciliation (identify safety risks and monitoring needs) Assess and set monitoring plans for: Skin integrity / wounds (baseline, red flags, escalation triggers) Catheters (baseline care, infection prevention cues, escalation) Complex medication routines (adherence, side effects, high-risk indicators) Provide escalation guidance and documentation standards for caregivers Communicate findings to families and (as needed) coordinate with external providers Participate in occasional caregiver coaching (brief, practical guidance)
Requirements
Required Qualifications
Active RN registration and in good standing with College of Registered Nurses of Alberta Minimum 3+ years RN experience (community/home care preferred) Strong assessment skills, clinical judgement, and documentation Valid driver’s license and reliable transportation (home visits) Vulnerable Sector Check (or willingness to obtain)
Assets (Nice to Have)
Wound care experience (basic/intermediate) Catheter care experience (Foley/SPC) Geriatrics, dementia care, falls prevention Case management / care coordination background
Compensation Model
Per assessment fee + paid documentation time, or Hourly (including travel/documentation) (We pay for documentation time—no unpaid charting.)
