• Procedures/Duties:
- Routine medical evals and chronic disease management
- Annual wellness and preventive care assessments
- Management of dementia, frailty, and other complex chronic conditions
- Oversight of palliative and comfort-focused care plans when appropriate
- Review and approve admissions to SNF, incl completing comprehensive H&P exams.
- Conduct physician compliance and recertification visits
- Provide medical care for long-term care residents
- Develop, review, and update individualized care plans
- Manage acute changes in condition, evaluate new symptoms
- Prescribe, monitor, and adjust medications while reviewing consultant pharmacist recommendations and ensuring medication safety.
- Participate in interdisciplinary care conferences, family meetings, and goals-of-care discussions as appropriate
- Complete required documentation, physician orders, certifications

Requirements:
• Active MT License or IMLC
