Description
Franciscan Ministries in Baltimore, MD seeks a compassionate Memory Care Manager to lead person-centered services for adults with developmental disabilities and memory-related needs. In this role, you will oversee daily operations of memory care programs, supervise and coach direct care staff, and ensure individualized support plans promote dignity, safety, and independence. You will coordinate health and behavioral supports, partner with families and clinical providers, and maintain regulatory compliance and documentation. The ideal candidate brings strong leadership, dementia care knowledge, and a commitment to faith-inspired, community-focused service.Oversee daily operations of memory care services for adults with developmental disabilities.
Develop, implement, and review individualized support and memory care plans.
Supervise, schedule, and coach direct care staff to ensure high-quality, person-centered support.
Coordinate with healthcare and behavioral health providers to address resident needs.
Ensure compliance with state regulations, agency policies, and documentation standards.
Monitor health, safety, and risk, implementing proactive interventions and improvements.
Engage families and guardians as collaborative partners in person-centered planning.
Lead staff training in dementia care, safety, and best practices.
Track program outcomes, quality indicators, and make data-informed improvements.
Support and model the organization’s faith-inspired, dignity-focused culture.
