Minimum Requirements:
Master's degree in a related field and minimum of two (2) years post graduate work experience. Additional experience may be used on a one-to-one basis to offset the educational requirements. Must have expert level experience and comfort with technology, including working with others on the use of technology in order to problem shoot possible technology issues during remote trainings. Position Description:
The Trager Institute is seeking a dedicated and compassionate Social Worker II to join our team to make a meaningful impact on the lives of older adults. Responsibilities include ensuring programs align with the latest evidence-based practices and meet the specific needs of individuals with various health issues. Job duties focus on care coordination for older adults, addressing social determinants of health, and delivering education and training to patients, caregivers, and supportive care workers.
This position is grant funded and is contingent upon the renewal of those funds.
Essential Duties and Responsibilities
- Coordinate case management services helping bridge the gap between clinical care and community resources.
- Provide support to older adults and their families helping them navigate healthcare systems and access community resources.
- Advocate for the needs and rights of older adults within the healthcare system and community.
- Supervise and mentor social work students during their field placements, providing guidance, feedback, and professional development opportunities.
- Participate in data collection methods and work closely with the data team to ensure outcomes are being met.
- Deliver comprehensive training programs on dementia care for healthcare and supportive care workers, enhancing their skills and understanding of Alzheimer's Disease and Related Dementias (ADRD).
- Monitor and evaluate the effectiveness of dementia care programs, making adjustments as needed to improve outcomes for individuals with ADRD.
- Ensure all programs and services comply with relevant regulations and standards, maintaining high-quality care for individuals with ADRD.
Preferred Qualifications
- Previous work experience with members of the community and older adults, understanding of older adult needs and healthcare needs, knowledge of various local community resources and services.
- Strong understanding of age-friendly healthcare principles and practices.
- Experience with data collection and reporting is preferred.

- Excellent interpersonal and communication skills to communicate effectively to build relationships working toward a desired outcome.

Target Compensation Maximum:
$70,623.00
Target Compensation Minimum:
$47,048.00
Compensation will be commensurate to candidate experience.
