Description
American Traveler is seeking a licensed Physical Therapist with home health experience for a 13-week home health assignment in Kingman, AZ.
Details
- Home health setting serving patients in the Kingman, AZ area
- Patient diagnoses include CVA, heart failure, Type II diabetes, post-amputation, Parkinson's disease, paraplegia, and dementia
- Monday through Friday day shift, 8:00 AM – 4:30 PM (5x8-hour shifts)
- No on-call requirements
Requirements
- Minimum 1 year of physical therapy experience required
- Home health or outpatient PT experience required
- Current AZ PT license required (must be in hand at time of consideration)
- Current BLS certification required
- Level 1 fingerprint card required
- Two professional references required for consideration
- PDF copies of driver's license and Social Security card required for consideration
